Carers Matter Norfolk, Young Carers & Families is a partnership led by Voluntary Norfolk, delivering support to young carers and their families in Norfolk. The service focuses on removing or reducing caring delivered by children or young people to help them have the same life opportunities as their peers.

The Family Support Worker will support and empower the whole family to bring about change to the caring situation with practical support, emergency planning, peer support, information, advice and guidance. The team work to a Whole Family Approach which engages with the wider network surrounding the young carer and their family.

Main Duties

1. **Whole Family Assessment and Support Planning**
- Working to a flexible delivery model, undertake holistic assessments of young carers and their family’s needs.
- Work with the family to assess their strengths, support networks and what matters to them; assisting them to identify and articulate their desired outcomes. Taking into consideration the wider family network, co-produce an action plan based on these outcomes.
- Empower families to achieve their goals & outcomes identified in their assessment and action plan.
- Measure outcomes, capturing different aspects of the family’s life circumstances and their journey, completing reviews, measuring progress, achievements and sustainability against their goals.
- Plan and record, with the young carer and their family, a Young Carers Emergency Plan, ensuring those in the family’s wider network are aware of the plan and any responsibilities they may have as part of it.
- Where there is an identified need for either a Care Act Assessment, Young Carers Needs Assessment or an Adult Carers Assessment the worker will support access of these assessments by making necessary referrals and continue to support the family through their journey.
- Support empowerment, independence, social and emotional wellbeing through information, advice and local opportunities. Encourage use of existing community resources and facilities.

**2. Community Networks, Peer Support and Capacity Building**
- Maximise support for families through effective partnership working and improved referral pathways to voluntary and statutory services
- Attend regular Early Help Hub collaboration meetings to develop professional networks; and draw on a multi-agency approach.
- Work with Caring Together to support school involvement and awareness of young carers and the challenges they face.
- Support at information and awareness events.

**3. Record Keeping**
- Record all interactions, interventions, notes, assessments and actions for follow-up on the Client Relationship Management (CRM) database in a timely manner and to expected quality standards, ensuring data is accurate and proportionate.
- Ensure that information, including personal sensitive data, is collected, processed, and stored according to information governance best practice and the standards required by Voluntary Norfolk (VN) policies.

**4. Performance and Development**
- Deliver against service Key Performance Indicators and specific targets.
- To attend regular team meetings and team hubs.
- Positively participate in staff development, including service monitoring and evaluation, coaching/training and personal development planning, supervision and appraisal, in line with the performance management framework.

Pay: £26,080.00 per year
