This new role as a Connect Professional is based within the Older Adult CMHTs. The aim of this service is to help increase access to appropriate services for Older Adults which includes early identification of dementia and using adapted evidence-based interventions which meet mental health needs and frailty. This involves linking in with the Mental Health Neighbourhood Teams, Primary Care and the voluntary sector organisation. This post does not carry a caseload as the role is designed to work with and alongside others and provide education and liaison.
