Saxmundham Health have an exciting, new opportunity for a Care Co-ordinator to join our Primary Care Network. The role is based at Saxmundham Health Centre.
**Main duties of the job**
The role can be offered on a full time or part time basis
The role involves working very closely with the practice and the multidisciplinary team
(MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.

Saxmundham Health is both a forward-thinking but also more importantly ‘forward-acting’. We have daily Physio presence and 10 sessions a week of Mental Health nursing.
Saxmundham Health is a friendly, supportive and dedicated team delivering high quality healthcare while putting the buzz back into General Practice.
We are working on developing a One Stop Shop for a site which already has full Planning Consent for an NHS Nursery, Café, Social Care Hub, Out of Hours, Ambulance and a Training and Education Centre.
Saxmundham Health is part of North East Coastal PCN (Primary Care Network) that provides healthcare services to 26,000 patients across rural Suffolk. North East Coastal is headed by 3 proactive, supportive and committed Clinical Directors. The PCN includes the following practices: Saxmundham Health Centre, Leiston Surgery and Framlingham Surgery.
We achieve consistently high QOF and CQC results. We are active and respected members of Suffolk and North East Essex Integrated Care System. We provide an enhanced health in care homes service to two Care Homes we have in our catchment area.

**Responsibilities**:
The Care Coordinator’s role will support the practice in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will include supporting digital initiatives and includes responsibilities for the coordination of the patient’s journey through primary care.
- To assist the doctors and other clinicians in the provision of a high quality service.
- Answering general enquiries, making new and follow up appointments.
- Seeking patient information and results from hospitals and other organisations.
- Giving results to patients under clinical direction.
- Contacting hospitals to follow up referrals.
- Liaise with hospitals for updates on patients or advise of any relevant changes in patients’ condition as directed by clinicians.
- Completing and submitting quality alerts as appropriate.
- Dealing with requests for information
- Organising & attendance at MDT meetings.
- Taking of notes at MDT meetings & preparation of associated minutes.
- Booking of follow up appointments for patients that are requested by clinicians.
- Supporting reception team in dealing with transport requests.
- Reviewing, filtering and flagging information on Workflow requiring urgent action to clinicians.
- Contacting/chasing patients as requested by the Clinicians.
- Pro-actively ensuring that booked appointments are appropriate & with the correct clinician.
- Overseeing reports and prompting Doctors when urgent.
- Look up results on the system or chase with hospital/lab
- Assisting clinicians with Care Plans.
- Coordinating & actioning admin aspects of care plans.
- Undertaking quality improvement and change in practice.
- Assist with audits as required.
- Understanding the importance of medical terminology, data and coding
- Record keeping with use of appropriate coding.
- Understanding vulnerable groups and long term illness.
- Use of patient information resources
- To work as a positive and active member within the Practice Team.
- You will be expected to take part in an annual appraisal at which your performance will be reviewed and your training needs discussed.
- Attend relevant meetings as required including staff meetings. You may be requested to attend other meetings at the request of the medical or management team.
- Supporting, supervising & mentoring others
- Participate in peer learning with other Care Coordinators within North East Coastal PCN and nationally.
- Participate in any training programmes as agreed with Clinical and Managerial supervisors.
