This posting is for permanent positions at Sunlight Group Practice, based across our two sites; Parkfield Medical Centre, located in New Ferry & Allport Surgery, located in Bromborough.

This is an excellent opportunity for someone that is organised, enthusiastic and motivated and wishes to work in a team that is growing in order to maintain our high standard of care. The position of Health Navigator will provide a variety of skills and is ideal for those that like to be kept busy.

The duties include:

- Arranging appointments - internally and via the PCN platform
- Face-to-face engagement with the public & other healthcare professionals
- Handling calls from the public & other healthcare professionals
- Providing positive patient experience
- Assisting with patient queries
- Signposting to appropriate services
- Promoting the use of online consultation
- Referrals
- Targets/recalls
- Providing administrative support to the clinical team
- Demonstrating confidentiality at all times
- Liaising with secondary care, PCN, pharmacies and other services.
- Working within the guidelines and policies of the Practice.

Knowledge of EMISWeb and Docman 10 would be an advantage but not essential as full training will be given.

Pay dependant on experience.

**Job Types**: Full-time, Part-time

Pay: From £11.44 per hour

Expected hours: 16 - 33 per week

- GCSE or equivalent (preferred)

- Administrative experience: 1 year (preferred)
- Customer service: 3 years (required)
