An opportunity has arisen for an infection prevention Case Worker for Client Affairs based out of Capital House in Winchester with flexibility to largely work from home.We are looking for a motivated and enthusiastic individual who has an understandingof the mental capacity act and a passion for social care to come and join us. The main objective being following the impact of Covid and adhering to strict infection control guidelines, to visit vulnerable people largely in care home settings, many classedas clinically extremely vulnerable, assess wellbeing, and complete reports. Where infection prevention advice is given - escalation is then required re outbreak management with public health colleagues.Service users will need to make a safe transition backto connecting the community. Some service users will have limited insight into keeping themselves 'COVID' safe particularly with needing support to understand social distancing and adhering to infection control measures and related COVID restrictions withinmore independent supported accommodation arrangements.The successful candidate will have great teamwork skills, but equally be motivated to work alone, coming from both an administrative and social care background, have excellent IT skills with knowledge ofsocial care matters and the impact that Covid 19 has had on residents to help shape this new role. You will visit people who lack capacity to manage their own finances, checking their wellbeing, advising staff on infection prevention control care plan, familycircumstances, access to personal funds where appropriate, identifying activities, liaising with them and their immediate loved ones or carers who reside with them. This will be recorded in a prescribed form, completed as soon after the visit as possible withkey concerns fed back to the relevant case officer in Client Affairs.With support from senior colleagues, you will plan your visits geographically, according to need and demand. There is an added requirement to reach out to some hard-to-reach isolated individualsin their own homes, providing community-based support, advising on infection control and access to finances to secure access to their community. Additional support for Service users to minimise risk of losing tenancy as a result of mental health needs andmental health crisis. Increased incidents and referrals are being received for people presenting in services where increased vulnerability is illustrated by deteriorating mental health presentations, unpredictable lifestyles, influence of drugs and alcohol,limited engagement with services, risk of tenancy breakdown through poor adherence to COVID restrictions/ tenancy enforcement.The team is based in Winchester, but this role will cover the whole county of Hampshire including Southampton. The successful candidatewill need to be a car driver with access to transport for business purposes.
