An opportunity has arisen to work across an integrated dynamic, innovative and friendly team within Wirral Community NHS Foundation Trust Integrated Discharge Team (IDT).
The team is going through a very exciting period of transformation within the trust and an excellent opportunity has arisen for a position within the IDT team. We are looking for an enthusiastic and forward thinking Care Navigator to join our busy, pro-active team based at Arrowe Park Hospital on a fixed term basis, with the potential opportunity for this to be extended.
The role will involve assessing and working with people with a wide variety of both physical and mental health conditions as well as complex social issues. This role will involve collaborative working and taking responsibility within our experienced multi-disciplinary teams - MDTs, to ensure best possible outcomes are achieved for people using the service, their carers and families. You will attend regular MDT meetings and ensure that you work effectively to provide person - centred care, ensure a timely journey for people who use the service, prevent discharge delays and avoidable admissions to hospital and long term care.
You need experience of working in a Health or Social Care environment role and have an excellent understanding of current social work legislative practice. There will be a need to ensure that you work in compliance of the requirements of The Care Act 2014. Risk management, excellent communication skills and an ability to prioritise and often work at a fast pace is essential. You will need good time management skills.
This post requires applicants to work alongside Social Workers in completing non-complex assessments, reviews of short term placements to identify long term care needs, MCA and Best Interest Meetings where applicable. It is also essential that the post holder is able to be effectively resilient to challenges posed by the Acute Trust (hospital) during winter pressures.
The post holder will be required to actively participate in quality improvement initiatives and to ensure that they manage their continuous professional development. There will be the opportunity for regular supervision and to attend in-service training and professional development.
- To work within the integrated Health and Social Care teams based within the Gateway.
- Rapid Community Response and the Transfer To Assess bed based services.
- To support the team by providing statutory reviews to identify on-going eligible needs and long term care provisions. (The Care Act 2014), following a period of Rapid response and or a Reablement stay of up to 4.2 weeks within the bed bases services.
- To support people through a reablement pathway in a timely and efficient manner, ensuring the best outcomes are achieved for the individuals, to maximising independence and improve Health and Wellbeing at all times.
- To be proactive and liaise with the social work team to gather feedback on the clients progress and review needs throughout the Rapid Response and Transfer to Assess service pathway.
- To specialise in assessment, ordering and installation of low level equipment and to facilitate safe use support and guidance for clients, staff, families and carers and other professionals where necessary.
- To work jointly with commissioned provider agencies and commissioners in WBC and WCNT.
