Are you passionate about supporting vulnerable adults in our community? Guild Care is seeking a compassionate and dedicated Take Home and Settle Support Worker to join our Community Services team.

**Essential Requirements**:

- Experience working with vulnerable adults
- Willingness to work weekends (Saturdays) on rotation
- Knowledge of health and prevention issues for older individuals
- Understanding of low-level interventions in promoting well-being
- Commute within 20 minutes of Worthing Hospital during work hours
- Ability to manage own emotional responses and maintain confidentiality
- Proficient in customer service and communication skills
- Satisfactory DBS check
- Current Driver’s license and access to a car

**Desirable Skills**:

- Team player with internal and external agencies
- Familiarity with Primary Care, Social Care, and Voluntary Sector services
- Experience working with carers
- Knowledge of Community Services in Worthing and Adur area
- Health & Safety qualification
- Care or First Aid qualification

**Your Role**:
As a Take Home and Settle Support Worker, you'll play a crucial role in helping vulnerable adults return home safely after attending A&E, preventing unnecessary hospital admissions. Your responsibilities include assessing customers' situations, making referrals to other agencies, and ensuring their safety at home. You'll also provide support to those discharged from hospital, promoting their well-being and independence.

**Key Responsibilities**:

- Keep the customer at the centre of all activities
- Respond to concerns about customers' physical or emotional health
- Provide information, advice, and practical solutions
- Encourage self-care and lifestyle improvements
- Conduct consistent assessments and report major concerns promptly
- Collaborate with professionals and service providers
- Maintain accurate records and adhere to policies and procedures
