Making A Difference as an Assessment and Review Worker in DOLS and CHC Team
We are currently looking to recruit an Assessment & Review Worker into the DOLS and CHC Team. The team’s base is Forster House and consists of Social Workers, Best Interest Assessors and Assessment and Review Workers.
The role of the Assessment and Review Worker is to arrange and manage reviews for individuals who receive support provided by the Local Authority and ICB. This will include individuals supported within a variety of settings including in their own home, in residential or nursing care, within sheltered accommodation and supported living settings. The reviews will ensure the support provided continue to meet the individuals care needs as defined under the Care Act 2014. Other tasks/responsibilities required within this role would be to assist the team by carrying out a Duty Role according to the needs of the team and to undertake joint visits with other team members where necessary.
This is an excellent opportunity to gain and develop experience. The team is a supportive team and the Assessment and Review Worker will work alongside Social Workers and Managers. In addition there will be an opportunity to work closely with colleagues from the ICB. A good induction and monthly supervision will be provided as well as ongoing peer support.

The team has a flexible working arrangement, which means you will be required to divide your working week between home and the designated office
