Maternity Tobacco Dependence Advisor

Job description Communication Promote and maintain good working relationships with all grades of staff and the multi-disciplinary maternity team through effective communication. Maintain effective communication channels with clients taking account of their individual needs. Communicate all relevant information to the multi-disciplinary team internally and with external organisations as required to ensure seamless and safe care. Regularly communicate with the Tobacco Dependency Lead Midwife, particularly in regards to complex cases, seeking advice if and when necessary.
To liaise with others in the health and social care community in order to promote the stop smoking service and encourage appropriate referrals Responsibility for Patient Care Identify and approach women who are eligible for tobacco dependence conversations and treatment using a variety of different methods and within a range of settings. These may include telephone, in hospital, virtually, in a persons home or in a community clinic. Follow women throughout their engagement with the service to the location of their care provision in order to integrate care. To plan and deliver stop smoking advice and support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines Participate in educating pregnant women about the benefits of stopping smoking in pregnancy and give support to those women wanting to quit.
Promote smoke free homes, and support andadvise women and families about second hand smoke. Provide general health, lifestyle advice and support to people using the service e.g. healthy eating, physical activity, stress management. This will include individual and group-based interventions, signposting and referring to other health, community, and voluntary services.
Deliver Carbon Monoxide (CO) monitoring to a high standard and engage in the data collection and collation of CO rates in pregnancy and post-partum. Planning/Organisation Once all training has been completed, carry a caseload of patients appropriate to your level of experience, confidence and competence. Respond effectively to referrals within stated timescales Be responsible for managing your own time including scheduling appointments with patients, being as flexible as far as possible over the time and location of the appointments and act within monitored set timeframes whenever possible. Develop individually tailored support plans including recommending treatments based on assessed needs.
Responsibilities for Physical and / or Financial Resources Ensure that relevant equipment is provided and maintained on an agreed schedule. Report any failings of equipment or other environmental factors to the Tobacco Dependency Lead Midwife. Ensure stock is regularly replenished in all ward/department areas Monitor stock levels of equipment, sundries, resources etc and inform Tobacco Dependency Lead Midwife when supplies are low Responsibility for Policy and Service Development and Implementation Seek out opportunities to develop and implement stop smoking services in a range of health and community settings. To be involved in the monitoring and evaluation of client feedback, offering recommendations as part of the team to improve services.
Contribute actively to open discussions of how local services can be improved, supporting improvements within the local community, and demonstrating an understanding of the barriers individuals are experiencing. Be aware of how to report incidents, concerns or complaints and escalate to the Tobacco Dependency Lead Midwife Responsibilities for Human Resources and Leadership The post holder will be required to complete all mandatory training and appraisals on an annual basis. Responsibilities for Teaching and Training To support the delivery of tobacco dependence training to the multidisciplinary team Assist and support new members of staff and students Undertake relevant training to maintain the skills and knowledge as necessary, through participation in continuous development Responsibilities for Data and Information Resources Engage with the Smokefree Pregnancy Incentive Scheme, keeping accurate records and enable good data collection. Maintain accurate up to date and comprehensive records at all times. Enable utilisation of the Smokefree Pregnancy Data Platform, for recording patient data, outcomes and for reviewing own personal performance against programme KPIs.
Research, Development and Audit Play an active role in the development of the Smokefree Service targets and work collaboratively with team members to share best practice and information to achieve these.
