Patient Safety Co-ordinator

To work as part of the Patient Safety and Governance Team, supporting the Trust’s transition to the Patient Safety Incident Response Framework by providing key administrative functions including, monitoring trackers within the local incident management system (DATIX), facilitating the process for receiving and acknowledging CAS alerts, assisting with the learning from deaths process, patient safety improvement data collection (Sepsis/AKI), general administration of meetings and quality assuring internal reports.
Upload Patient Safety incidents to external bodies (currently National Reporting & Learning System - NRLS) in a timely manner
Facilitate and monitor the process for the safe receipt and closure of alerts sent via the Central Alerting System including National Patient Safety Alerts.
Receipt, collation and presentation of data to support Sepsis/AKI improvement practitioners
To produce accurate high-quality communications and correspondence, often of a distressing and emotional nature, letters and other documents, many of which are non-routine as required using audio/copying typing and word processing systems as required
Receipt, collation, and provision of relevant information in response to complex information sharing requests under the Freedom of Information Act 2000 and GDPR, where required.
Undertake ad hoc work on relevant projects as directed by the Head of Patient Safety Improvement
Quality assure internal reports
Support the Patient Safety and Governance team with the daily review of reported incidents.
To assist, develop and maintain platforms for communicating shared learning, Trust intranet and newsletters

The post holder will be a key member of the Patient Safety and Governance team, supporting the team as well as staff within divisions.
**CAS Alerts**:
Facilitate and monitor the process for the safe receipt and closure of alerts sent via the Central Alerting System including National Patient Safety Alerts.
**Incident Trackers**:
Facilitate the process for the trackers that are used to monitor Trust safety investigations.
**Learning from Deaths**:
Facilitation, co-ordination and monitoring of the Trust’s Learning from Deaths process.
**National Reporting**:
Upload Patient Safety incidents to external bodies (currently National Reporting & Learning System - NRLS) in a timely manner
Receive and respond to information requested from the ICB and CQC
Complete local, regional and national Benchmarking submissions as required
**Patient Safety Improvement**:
Receipt, collation and presentation of data to support Sepsis/AKI improvement practitioners
To provide administrative support to the Trust’s Deteriorating Patient Operational Group.
**General Admin duties (not exhaustive)**:
To provide administrative support function to the Patient Safety team including information and actions being added to the Trust’s incident reporting system.
To support the Band 3 administrators with the administrative function of investigations ensuring identified incidents are appropriately allocated, facilitated and monitored to achieve compliance with strict timeframes.
To work with Divisional representatives and the Lead Patient Safety Investigator/Learning from Deaths Lead to monitor and facilitate assurance requests arising from reviews
To develop and produce accurate high-quality data presentations and reports using computer software, in a format which is clear and relevant to the report required.
Provide information which is relevant and accurate which contributes to effective management of patient safety.
Undertake ad hoc work on relevant projects as directed by the Head of Patient Safety Improvement
Support the Patient Safety and Governance team with the daily review of reported incidents.
