**43.75 hours per week on rolling 2 week rota**

**Shifts: 07:30 - 20:00**

**Week 1: 3 days**

**Main Responsibilities of the Support Worker**:

- Offer advice, information, and support to clients, including referrals for aftercare focused on maintaining an abstinent lifestyle.
- Assist Therapists with risk management plans and reviews.
- Ensure clients follow program rules, supporting the manager in investigating breaches, including substance testing.
- Conduct intakes, assessments, and discharges efficiently.
- Keep accurate client records and maintain databases.
- Facilitate transfers between agencies and services for clients.
- Support clients with medication reminders, conduct audits, and maintain accurate records.
- Work with clients to create recovery plans that are realistic and achievable.
- Help clients attend external appointments in line with their care plans, providing transport support or accompanying them as needed.
- Assist with day-to-day operations, helping clients manage practical aspects of daily living such as life skills, housing, and benefits.

- Strong organisational and time management skills.
- Clear verbal and written communication at all levels.
- Ability to work independently and as part of a team.
- Proficiency in Word, Outlook, and Excel.
- Excellent listening skills.
- Ability to work safely and effectively on your own.
- Strong negotiation and boundary-setting skills.
- Ability to create and review risk management plans.
- Empathy and a non-judgemental, calm approach.
- Capacity to handle high-pressure situations.
- Great customer service skills.
