**Preparing for Adulthood Link Worker**

**About the Role**

There is a new opportunity for 3 Preparing for Adulthood Link Workers in Cornwall (one for each locality, East, Mid and West) working alongside the Transition Social Workers to support young people between the ages of 16 to 25 who are in transition from Children’s Services to Adult Social Care.

Whilst the location/work-base for this role may have been specified our evolving ways of working means that the role may require you to also work remotely (e.g., from home/other locations). We will discuss this with you during the recruitment process.

**About You**

You will work alongside young people with varying and complex needs and across a range of settings, undertaking Transitions Needs Assessments and Reviews in line with the Care Act 2014. The main focus of your role will be to work in partnership with the young person, their family and carers by developing a relationship to support and advise the young person to identify their specific needs and outcomes as identified in their Transitions Care and Support Plan and within their EHCP in preparation for their transition into adulthood. In partnership with the young person and those supporting them you will identify their aims and aspirations and help them to achieve these as they navigate their transition into adulthood.

Recognising that needs and aspirations can change over time; you will need to show adaptability and adopt a person-centred approach to support maximising the young person’s potential and opportunities upon reaching adulthood. You will work with the young person and their support network to assisting them to build the right support. This will require you to be resourceful, dynamic and have the ability to engage the young person in exploring and developing their local community and wider networks.

You will work collaboratively with a range of partners who work with young people with special educational needs including SEN workers, children’s social work teams, health professionals, adult social work teams, advocates, family and carers in order to provide a seamless service to young people transitioning into adulthood.
