We have a large community therapy team across a number of localities within East and North Hertfordshire who work with a variety of different patients. We are looking for a physiotherapist or Occupational Therapist to join our forward thinking and innovative team to continue to provide excellent rehabilitation to patients within their home environment. The post holder has the opportunity to join the discharge home to assess service.

Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation.

The therapist will work alongside occupational therapists, therapy assistants, social workers, community nurses, GPs and care staff. Staff meet regularly for professional development and peer support, the post holder will have the opportunity to supervise and delegate tasks to junior staff and offer placements to physiotherapy students

This post is full time but applicants who wish to work part time or flexibly will also be considered.

The service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend rota.

The service covers both East and North Hertfordshire and the post holder may be expected at times to cover across all localities.

You must hold a full valid driving licence and have access to a care to use regularly for business purposes are essential (unless you have a disability as defined by the Equality Act 2010).

- To be part of an integrated health and social care team. To provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge.

**MAIN DUTIES and RESPONSIBILITIES**:
Operational
- To be professional, responsible and accountable to HCT and your professional body for all aspects of own work. This includes the management of patients in your care, and for any treatment plan prescribed.
- To maintain a high standard of clinical care for patients under your management and to complete advanced risk assessments independently within the patient’s own home.
- To formulate and deliver an individual Therapy management and treatment programme, based on evidence based practice and expert clinical knowledge. Using advanced clinical assessment, clinical reasoning skills and knowledge of treatment techniques. This may include functional training, behavior modification, patient education, fatigue management/energy conservation.
- To evaluate patient progress, reassess and alter treatment programmes if required
- To assess and manage clinical risk within own patient caseload, and to report to other professionals as appropriate.
- To be responsible for ensuring the effective selection and use of all treatment resources available.

Patient / Customer Care
- To undertake advanced, comprehensive assessments using investigative and analytical skills.
- To assess patient understanding of treatment proposals, gain valid informed consent and work within a legal framework with patients who lack capacity to consent to treatment.
- To participate with individuals and groups on health promotion.
- To provide information and updates to nursing staff and other members of the team regarding a patients functional, and cognitive abilities

ServiceDevelopment and Improvement
- To comply with the organisational and service policies and procedures and to be involved in reviewing and updating as appropriate.
- To identify improvements in service delivery and clinical practice, including the implementation of relevant aspects of the national service framework and other local/national initiatives.
- To be aware of health and safety at work and implement any policies which maintain or improve standards; to record and report incidents promptly to senior staff.
