**Remit**:This role acknowledges the high prevalence of mental health issues in East Durham and the impact on Carers’ own health and well-being. It aims to enhance the identification of Carers and improve referrals to DCCS by strengthening collaboration with partner agencies. Additionally, the role supports Carer Support Coordinators in East Durham by sharing up-to-date information on available services to assist Carers or those they care for who are experiencing mental health challenges

**Main tasks and responsibilities**:

- To identify, communicate with and give presentations to agencies/ staff teams, championing the role of Carers and explaining the business case for identifying and referring to DCCS.
- To build on existing lines of communication to maintain and improve knowledge across all sectors of current issues and challenges experienced by Carers.
- To work with the digital communications coordinator to improve communication with staff teams digitally and via a quarterly professional bulletin.
- To use local knowledge and DCCS monitoring reports to plan organisations and teams to target, due to low levels of referrals historically.
- To develop partnerships and collaborations with local Organisations, charities and health and social care providers
- To build on links made with Primary Care including ICB, PCNs, GP practices and SPLWs to ensure good communication and identify any opportunities to influence services.

**Job Types**: Part-time, Fixed term contract

Pay: £11,050.00 per year

Expected hours: 14 per week
