**Job summary**:
To maintain a high standard of professional practice in the assessment of care needs, treatment and evaluation of programmes of care. To undertake initial team assessments of referred clients and report to the referral agents after discussion/supervision from the appropriate Team Meeting. To also undertake joint assessments with colleagues from other disciplines and agencies as and when appropriate. To formulate and implement treatment programmes and report to the referring agent on the client's progress. To complete an accurate assessment of risk with all new clients and provide the initial management plan. All risk management plans will be reviewed and updated as appropriate.
To participate in the audit of clinical work to focus in particular on quality and outcome issues.
To have an appropriate understanding of legislation relating to work in the psychiatric setting, and in particular the Mental Health Act 1983, and all other relevant legislation.
To combine with other members of the team, either in conjoint therapy or case work as appropriate.
Where appropriate, to communicate and liaise with other agencies involved in providing health care and social care.
To contribute to multi-professional assessment and treatment.
To liaise with ward staff/crisis team and provide care co-ordination to clients in hospital.
To abide by relevant Professional Code of Conduct.

**Main duties, tasks & skills required**:
a) To promote and maintain a close working relationship with medical staff, other nurses and therapists.
b) To assist in establishing and maintaining lines of communication with wards, and hospital departments, G.Ps, voluntary agencies and other organisations.
c) To be aware of and liaise with therapeutic and supportive facilities in the community as appropriate.
d) To continue to support carers and relatives and engage in carer support and family work interventions.
To contribute to multi-professional assessment and treatment.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
