**Care Coordinator**

**Peterborough, Cambridgeshire**

**£23 - £27k Basic + Oncall £3,100 per annum approx. (based on completing on-call every 5 weeks)**

**Your working hours will be between 09:00 -17:00 Monday - Friday.**

The service we operate is specialist and Nurse Led, we provide highly trained care teams to work closely with clients in the community. We rely on Healthcare Assistants, Support Workers and Registered Nurses to support our service users within their ownhomes. Behind the scenes in the East operation, we have a team of staff who work to co-ordinate and manage all aspects of our service.

**Applicants must hold a UK Driving Licence**

**Main responsibilities as a Care Coordinator**:

- Building relationships with commissioners, professionals, clients and family members
- Rota Management for your packages of care
- Responding efficiently to day to day changes in your care packages
- Providing excellent customer service
