**Responsibilities**
The following are the core responsibilities of the Paramedic Practitioner. There may be on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
- Provide the first point of contact for clinical queries from the practice reception team
- Be responsible for telephone triage
- Undertake urgent/same day clinics
- Undertake home visits alongside the GP visiting team
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patients needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan
- Initiate and interpret tests and investigations as appropriate
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols within scope of practice
- Work with patients to support compliance with and adherence to prescribed treatments
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support and manage health needs of women presenting for family planning or sexual health consultation
- Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
- Record accurate data in the patients records to an agreed standard
- Collaborate with other members of the Primary Health Care Team including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
- Direct referral to and liaison with consultants and other health professional agencies
- Ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge.
**Quality Requirements**
- Recognises and work withing own competence and professional code of conduct as regulated by the HCPC and Royal College of Paramedics
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures
- Be aware of data protection (GDPR) and confidentiality issues particularly within a GP surgery
- Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information
- Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes
- Prioritise, organise, and manage own workload in a manner that maintains quality.
- Deliver care according to NSF, NICE guidelines, and evidence-based care
- Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation
- Keep up to date with current evidence-based practice
- Participate in research and utilise the audit cycle as a means of evaluating and improving the quality of work
- In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate
- Support and participate in shared learning across the practice
- Assist in the development of policies and procedures
