**Mental Health Practitioner (Special Interest - Severe and Enduring Mental Illness) **RMN/OT or Social Worker**

In the Hereford City Neighbourhood Mental Health Teams, we are working to improve the mental health and wellbeing of our local communities. Being an early implementer of the Community Mental Health Transformation project we have completed the alignment of our services with local Primary Care Networks and GP practices and have developed exciting partnerships with a range of voluntary services. We have developed new, local population focused, mental health pathways aimed at reducing health inequalities and improving the resilience of our communities.
You will work as a key member and resource of the core Neighbourhood Mental Health team. You will hold a blended caseload of patients with severe mental illness and those with common mental disorders, however your caseload will reflect your strong professional interest in supporting people with complex presentations related to severe and enduring mental illness and difficulties engaging with services. This post will also have opportunities to engage with local MAPA and MARAC processes and to provide specialist risk assessments for the locality team.
You will have experience of working with this user group in community settings. You will have a registered qualification in mental health nursing, occupational therapy or social work.
The ability to work flexibly as the need of the service demands is essential.

**Preceptorship for Newly Qualified Staff** - The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.
