**Responsibilities**:

- Deliver personal care and assistance with daily living activities
- Create and update care plans based on individual needs
- Communicate effectively with residents, families, and healthcare professionals
- Maintain accurate records of care provided

**Experience**:

- Previous experience in home care or a care home environment is desirable
- Proficiency in using IT systems for record keeping
- Ability to create and implement care plans tailored to individual requirements
- Excellent communication skills to interact with residents and colleagues effectively
- Fluent in English to ensure clear communication with residents and team members
