Aligned to the Integrated Discharge Hub, no two days will be the same and the opportunity to develop as a social worker as well as improve the quality of life of Lincolnshire residents is immense.
The Hospital Teams work across 7 days including weekends and bank holidays, the Hospital Teams are there to support Lincolnshire residents when they need them most.
The role carries a 15% Attraction and Retention payment

Do you want o expand your knowledge of working in both the Community and Hospital settings and teams aligned to the Integrated Discharge Hub? Then this 2 year fixed term contract is for you.
As part of the Hospital Service you will primarily follow Lincolnshire residents out of hospital to assess them in their own home to identify actual need and to support in maximising their independence. At times you may also be asked to work within the hospital settings to facilitate timely discharges, so that we can ensure that Lincolnshire residents return to where they want to be, in their own homes. Utilising a strengths-based approach this role will provide you with extensive opportunities to learn and develop.

You will be given a nominated contractual work base and you will be required to commute to this work base when necessary. Working arrangements including regularity of home and office working will be discussed as part of the appointment process._
