**Location**:North East (including HMP Durham, Holme House, Low Newton, Kirk Levington Grange, Northumberland) - the role will be field based and it will combine working within our prison sites, community hub and home.

**Salary**:£23,949 - £26,282pa

We have an exciting opportunity for **Specialist Health Care Navigator **to join our Specialist Healthcare Navigation (Enhanced Reconnect) service.

The aim of the **Specialist Healthcare Navigation (Enhanced Reconnect) service **is to reduce health related barriers that directly impact on the individual’s engagement prior to, and upon leaving prison; to reduce health inequalities and address the health and wellbeing of serious and high-risk offending behaviours. The service is a psychologically led service that offers interventions, structured support, advocacy and signposting to those leaving prison to engage with community
- based health and support services, reduce social exclusion and thereby reduce their health-related offending behaviours.

**How you will make a difference**

As a Specialist Healthcare Navigator, you will work seamlessly with the other health professionals involved in the service users care, and with other Care Navigators and Apprentice Health and Wellbeing Coaches, to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans. You will use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community you will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.

**What we are looking for**

We are looking for an individual who:

- is educated to NVQ Level 3 or equivalent with qualifications or training relevant to the role, e.g. Health and Social Care/Mental Health
- has relevant experience of supporting vulnerable individuals, to identify, set and meet their own goals using 1:1 and/or group interventions
- has a working knowledge of health, debts, social care, housing, court systems, mental health, criminal justice and substance misuse
- has experience of working to support individuals using structured, time limited support and am confident with this way of working
- is passionate and motivated to support individuals leaving the criminal justice system through provision of a high quality, professional level of care, recognizing and respecting individual differences and promoting diversity
- holds a full UK driving license and have access to my own transport for work purposes

You may also have:

- Experience of working in prison environment or experience of working with offenders.
- Experience and / or training in coaching
- Specialist interest and/or training in working with older adults, veterans, substance misuse, probation systems, youth offending or females who have been affected by the criminal justice system
- Mental Health qualifications or knowledge, including therapeutic interventions
