We are currently recruiting for enthusiastic and highly motivated mental health practitioners (RMN/occupational therapist/social worker) to join LYPFT’s Clinical Team of the Year covering:
- Veterans’ mental health complex treatment service (VMH CTS) - North West.
This exciting opportunity, within an innovative and experienced MDT and with our partner provider Combat Stress, is keen to recruit a forward thinking clinician with an interest in working with veterans. You will have post registration experience and be able to demonstrate an understanding of the needs of military veterans’ suffering combat related mental health problems within this community service to stabilise, treat and integrate veterans who have complex health needs.
This post is a temporary to cover maternity leave over a 12 month period.
You will be required to provide care coordination, utilising the CPA framework, for the delivery of care; maintaining a focus on recovery and adopting a person-centred approach. You will have excellent communication, organisational and clinical reasoning skills, as well as the ability to travel across the geographical region of the service.
You will deliver stabilisation interventions to veterans on your caseload in addition to on-going assessment of mental state and risk management, promoting the active involvement of veterans and carers in all aspects of their care delivery.

This job role description sets out in generic terms the role and competencies needed to be a band 6 care coordinator in the Trust. A more detailed job plan will be provided to the post holder specific to the Veterans Service.
The core purpose of this role is to co-ordinate the care of a defined caseload of service users from admission to the service and through to discharge.
The post holder will be highly skilled and competent in conducting clinical holistic and risk assessments (in accordance with the identified care pathway).
The post holder will provide a range of highly skilled interventions (including group work) as determined by the community mental health pathway (appropriate to professional expertise) and share those skills with other team members.
Whilst having access to good quality clinical supervision, the post holder will exercise a high level of professional and clinical autonomy.
The post holder will provide a high level of skill in supervision, education and skills sharing to other team members including learners
