**Primary Mental Health Clinical Team Lead B7: North Cornwall Coast PCN**
It’s a refreshing & exciting time to join our Adult Mental Health Services in Cornwall, as we are focused on enhancing our services with outstanding levels of quality care. Our transformation of our core community mental health service offer is well under way, and we are looking for creative people to join our new workforce. We are forging and renewing partnerships between colleagues in Primary & Secondary Care Services to create innovative and transformed mental health care for our population.
Due to these positive & transformative changes, we now have an exciting and unique opportunity for the right person to lead this transformation within North Cornwall Coast Primary Care Network under the employment of Cornwall Foundation Trust (CFT). The North Cornwall Coast PCN practices includes Port Isaac Surgery, Wadebridge & Camel Estuary Medical Practice and Bottreaux Surgery. Our practices are friendly, supportive, and already supporting a broad range of clinical staff focused on delivering a high standard of patient care.
The aims of this role will be to:
- Deliver improved access to a broader range of care options by offering timely and expert assessment.
- Aim to include evidence informed approaches that are known to impact on both mental and physical health.
- In keeping with the national guidance on the introduction of this role, it is envisaged that the focus of a practitioner’s work will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services.
- Reduce referrals to specialist services to enable earlier interventions that will improve patient experience and outcomes.
In a supportive partnership the PCN & CFT want to achieve care that is joined up, patient centred, trauma informed, place based (locality hub) where practitioners and communities collaborate optimising community assets. By reducing repeated assessments and addressing the needs of those with complex emotional needs earlier and locally, we can reduce distress and work with those with early adversity to have more meaningful and active lives, focusing on what matters to the person. This then enables secondary care to offer expert treatment to those with severe mental illness.
The mental health practitioner may be any registered clinical role including, but not limited to, a Community Psychiatric Nurse, and Mental Health Occupational Therapist. You need to be proactive approach to innovation and be passionate about providing a high-quality holistic approach to the care.
