**Contract**: 30 hours per week, permanent.

**Salary**: £21,507.75 pro rata (£17,206.20)

**Role Purpose**:
To deliver services to substance users aged 18 years and over to help reduce the harm caused by their substance use. The post holder will be required to deliver six structured one-to-one support to high-risk substance users then liaise with other service providers to refer on if required. The sessions will focus on reducing harm and risk, overdose awareness, health promotion, controlled substance use, trigger awareness, and managing recovery. These sessions will either be delivered at the agency or in the community depending on the need of the client.

The role will also involve contributing to a first point of contact service and facilitation a Needle and Syringe Programme.

**Responsibilities**:
Undertake substance misuse assessments reviews and risk assessments including risk management plans.

Take responsibility for referrals, care plans, and subsequent caseload whilst ensuring all records are accurate and up to date.

Work in collaboration with service users & develop and review care plans to meet identified needs.

Carry out individual one-to-one sessions Promote health awareness to individuals accessing the service and work in partnership with dedicated agencies to provide holistic support and assistance.

Facilitate Needle and Syringe Programme provision, providing appropriate advice to service users including Naloxone training.

Offer Dry Blood Spot Testing to those at risk of blood borne viruses.

Deliver crisis intervention through drop-in service.

Attend consultation meetings to address specific individual needs, to discuss referrals and progress.

Maintain appropriate databases and written records and undertake appropriate administrative tasks in a timely manner.

No qualifications are necessary for this role Experience of delivering services to substance users

Experience of working with the harm reduction model

A full UK driver's licence with access to a vehicle for work purposes
