**Paramedic Practitioner in Primary Care**:

**The Core Role Involves**:
**Daily Visiting**

Working for the PCN you will support the daily on-call team covering acute visits, across the three practices. The role will involve triaging the visits and then the assessment, diagnosis, treatment and where necessary, referral of patients. A large proportion of the role will be visiting patients in their own homes or care homes.

**Housebound Reviews**

We aim to work closely with the Proactive Care and Community Team to support our frailer population and provide vital Continuity of Care. There will be the opportunity to use or develop skills in monitoring long term conditions such as Asthma, Diabetes and COPD. People with those conditions at home (or in Care Homes) need reviews and the responsibility for these will be shared with other Paramedic Practitioners, GPs and ANPs in the practices.

**On The Day Care and Triaging**

The role will be diverse and will develop as your skills allow. The post holder must be able to demonstrate critical thinking in clinical decision making.

We would require you to have good communications skills, an ability to prioritise and manage your own workload, whilst being motivated by delivering exceptional patient care. It would be important that you recognise your own boundaries and scope of practice and to be able to seek the support and advice that will be available, when needed.

**Clinical Duties**:
The post holder will be expected to perform the following duties with the supervision of the On-Call Duty GP and will be required to respond to referrals and requests from various sources including:

- Patients
- Care Homes
- General Practitioners
- 111
- Out of Hours

Evaluate, diagnose and treat new and existing minor illness conditions.

Undertake home visits as required, assessing need for admission, drawing up holistic care plans to help patients to remain in the own home or care home, where appropriate.

Liaise with the patient's GP and the wider teams to plan ongoing care and monitoring including Anticipatory Care Plans.

Undertake and interpret diagnostics (including ECG's) and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use evidence based, clinical guidelines (NICE etc.) relating them to individual cases, aiding and feedback to the organisation and others on how these should be interpreted.

Notify GP of imminent End of Life medication requirements.

Signpost appropriately to other services in the identified Healthcare Pathway.

Refer patients to appropriate care settings, within locally agreed healthcare pathways.

Discharge or treat, as appropriate and within the scope of practice.

Maintain accurate and systematic documentation and electronic records of patient interactions and clinical care provided, including Adverse Events, Child Protection issues or their notifiable matters.

Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.

Provide clinical advice to other members of the team where appropriate.

**Professional**:
Work in partnership with the whole practice and PCN Team and participate and contribute to clinical meetings.

Work collaboratively with other members of the Health Care Team such as the Proactive Care and District Nursing Team, Mental Health Services, Care Home Staff, Ambulance Service, Secondary Care etc.

Ensure that the wellbeing of patients and staff is maintained in accordance with practice policies.

Undertake clinical audits in line with the organisation’s clinical audit plan and to participate as required in equipment testing.

Contribute to the analysis of information to inform future development of the service.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.

Work in a cost effective and efficient manner. Ensure the security, safe storage and maintenance of any allocated equipment and goods.

Identify risks to service provision and highlight them to the Clinical Lead.

Work within the requirements of Data Protection Legislation as it applies to patient records.

Engage in self appraisal and utilise reflective practice to enhance and develop personal and organisational practice.

Participate in continuing professional development opportuni
