This is an exciting post for a Physiotherapy leader committed to integrated working.

The East Integrated Rehab team provide services across two primary care networks (PCN) and two community hospitals in the area.

This post is central to fostering an effective integrated multi-disciplinary approach to meet the local health needs and the policy priority of developing services closer to home. The successful applicant will work closely with the East Occupational Therapy Lead and the other East Team Leads to develop integrated working that is aligned around PCNs.

To lead and manage the operational services of the Community Rehabilitation and Inpatient Physiotherapy teams in East Cornwall, considering local health needs and the policy priority of developing services closer to home. - Responsible for the delivery and maintenance of a high standard physiotherapy service including complex assessments, diagnosis, treatment and advice to patients, carers and affiliated services. - Responsible for creating an effective, integrated multi-disciplinary approach within the locality teams. - To ensure best practice guidelines and performance standards are met while meeting service targets. - To be an autonomous practitioner responsible for the assessment and treatment of patients with highly complex health and social care needs. - To provide developmental leadership and management to all staff within the locality. - To participate in the planning, development, and evaluation of therapy services, holding responsibility for defined projects. - Provide support to Integrated Community Manager (ICM), to accept delegated responsibilities for specific tasks. - The post holder will work alongside existing therapy team leads in North and East to influence and develop the future vision for therapy.
