**About You**

We are seeking to recruit a Health & Social care Coordinator to develop our efforts to increase health and social care access for Travellers in England. The Coordinator will play a key role within our team, working with families and health and social care organisations - as well as supporting our training programmes.

You will demonstrate a tangible understanding of the need for your role and be enthused about pursuing its purpose. You will show a vocation for service to the marginalised. You will be a first-rate communicator, have an ability to persuade and handle difficult conversations, with a talent for effectively managing multiple competing priorities. You will be adept at navigating public service systems as well as providing a compassionate, high quality service to vulnerable people, including in times of trauma and crisis. You will be a team player, but equally happy when working autonomously, possessing a good sense of humour and well-rounded humility. You will be warm and empathetic and also show professionalism and resilience.

**Health & Social Care Coordinator Responsibilities**
- Ensuring families and communities are registered and engaging with local GPs and hospitals.
- Supporting vulnerable individuals to access secondary healthcare or social care
- Over time, expanding our training programmes for healthcare providers and increasing good practice among CCGs and secondary care trusts.
- To build trust and confidence with the families and communities that you work with.
- To work with GP surgeries that are local to sites, ensuring that Traveller families are registered and receiving care at all stages of life to include preventative care.
- To disseminate Public Health information to communities.
- To work with individuals and families who have complex health needs, advocating for them, and helping them to navigate healthcare and social care systems.
- To participate in and follow our mental health training, practices, guidelines and referral systems, including being on the team supporting those who have mental health concerns.
- Working with healthcare providers alongside our CEO
- Developing and delivering training for healthcare providers on working effectively with Travellers and reducing communication/cultural barriers.
- Step by step, to encourage each CCG and each secondary care trust to have a clinician specialising in health pathways for Travellers.
- Build up our internal knowledge and external reporting on areas of good practice in healthcare practice in England and Wales. This may then lead to approaches to other health bodies about trialling such practices in their area.
- As your caseload may permit, to be willing to assist the MCT team in addressing wider issues within particularly vulnerable families - for example SEN, benefit forms and housing.
- Collect data and relay information to MCT to inform programmes and policies through our case work system within agreed timescales.
- To recognise needs that may require referral to other internal services or other agencies.
- To involve and empower family members throughout the service delivery; supporting
- them to find their own solutions, to engage with relevant agencies and to take the lead in their own lives and that of their families.
- To participate in relevant MCT induction visits, training and supervision.
- To understand and comply with MCT’s values, policies and procedures, including by executing MCT’s procedures for promoting and safeguarding the welfare of children and vulnerable adults.

**Essential requirements/knowledge**
- 2+ years experience in a role, working with the public, where your demonstrable skills can be transferred to this position (acceptable experience may include as a nurse, paramedic, public health professional, social worker, teacher, community worker etc.).
- Knowledge of basic medical information and of healthcare systems, particularly as they relate to the structure of the NHS in England.
- Hold a current valid driving licence and have the use of a car.
