Mendip Vale Medical Group are looking for a Social Prescriber to join our experienced North Somerset & South Gloucester team.

The post holder will be responsible for promoting healthy living to our patients, by providing expert signposting and support to engage patients with a wide range of community services. Supported by the Senior Social Prescriber you will be responsible for meeting agreed aims and objectives within a profitable, efficient, safe and effective working environment, ensuring the highest standards of customer service are delivered.

The Social Prescriber will develop good working relationships with:

- Patients
- Community services, 3rd sector
- Clinicians and the wider surgery staff
- Key stakeholders across the whole of BNSSG

**Key Responsibilities**:

- Co-Produce a simple personalised care and support plan to improve health & well being
- Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.
- Develop trusting relationships by giving people time and focus on ‘what matters to me’.
- Co-produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.

**Other Responsibilities**:

- Seek regular feedback about the quality of the service and the impact of social prescribing on referral agencies.
- Proactively encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach.
- To undertake any other duties commensurate with the post holder’s role as agreed with the Senior Social Prescriber

- Able utilise communication methods to disseminate information effectively.
- Able to adapt communication style and content to meet the needs of the user.
- Ability to receive sensitive or contentious information and process appropriately.
- Develop positivity and effective working relationships with health care providers and other key health care providers and other key partners to support an integrated approach to high quality patient care.

**Confidentiality**
- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
- Whilst performing the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, surgery staff and other healthcare workers. They may also have access to information relating to the Surgery as a business organisation. All such information from any source is to be regarded as strictly confidential.
- Information relating to patients, careers, colleagues, other healthcare workers or the business of the Surgery may only be divulged to authorised persons in accordance with the Surgery policies and procedures relating to confidentiality and the protection of personal and sensitive data.

- Report potential & actual risks identified, prepare mitigation and action plans to minimise risk to MVMG, its patients and staff.
