**Job overview**:
An exciting opportunity has arisen for an enthusiastic, innovative, flexible and well-motivated physiotherapy assistant to join the Newquay Rehabilitation Team based at Newquay Hospital. The role will be primarily supporting patients in their own home in the mid Cornwall area, although may also involve ward work when cover needed.

We deliver physiotherapy and occupational therapy in the mid Cornwall locality to individuals with a primary physical health condition in variety of ways:

- Community Rehabilitation Team - within the person’s home environment and intervention targeted at a wide variety of conditions, e.g long-term, neurological, orthopaedic, falls, frailty
- Intermediate Care - A more urgent service and working closely with the Home First/Early intervention team in the most appropriate environment for the individual.
- The Integrated Therapy team works closely with Specialist Therapy teams and is part of the East Cornwall Locality integrated Care Team working with Community Nursing, Social Care, Volunteer and Mental Health teams
- It will be an expectation of this post that on occasion, the person works flexibly across other areas of care.
- Being able to drive and having access to a vehicle are essential for this post
- Regular clinical supervision will be provided and continuing professional development encouraged
- We would welcome information calls or visits to the team
- Seven day working may be a requirement of this post in the future.

**Main duties of the job**:
The purpose of the post is:

- To undertake specific skilled physiotherapy and occupational therapy support work (e.g. carrying out assessments using various treatment skills) to aid the rehabilitation of patients.
- To plan and implement interventions, using various treatment skills and to monitor progress and evaluate the outcome.
- To encourage patients and carers in an active approach to regaining and maintaining personal independence.
- To support the physiotherapists and occupational therapists by carrying out timely reviews of rehab care plans.

Key Dimensions of the post are:

- To practise autonomously, under the supervision of a qualified therapist reporting back patient progress and informing therapist of any problems
- To hold own caseload of patients delegated by the qualified memb
