To utilise IT/information within the trust to facilitate better discharge on the ward and enable the ward to alter processes to improve discharge Manage the coordination of the more complex discharges from the ward ensuring ward NIC is kept up to date with any relevant changes or issues relating to that discharge The ability to input and provide accurate data and reports would also be desirable.
