**Job Purpose**:

- To provide support to vulnerable young people with a range of complex needs within residential or community settings across a range of shift patterns 24/7/365
- To use youth engagement and youth work skills to improve outcomes for children, young people and vulnerable adults by removing their barriers to inclusion and promote their active participation within the communities that they live in.
- To ensure the mental health and wellbeing of young people is supported effectively to enable children, young people and vulnerable adults to achieve a successful transition to adulthood.
- To develop their independent living skills and safeguarding them from societal neglect and abuse.
- To develop a boundary based, professional trusting relationship to motivate and engage children, young people and vulnerable adults in education, training and employment opportunities
- To ensure children, young people and vulnerable adults have the skills or the support they need to effectively manage their finances and have the necessary emotional intelligence/resilience and skills for the future.
- To work in accordance with principles of the Health and Social Care Act 2012, The Children's Act 1989 Guidance and Regulations, The Care Leavers Regulations 2010 and Planning Transition for Adulthood for Care Leavers 2011, as well as the SDA Care, Local Authority Safeguarding Policies and Procedures for Looked After Children and Care Leavers.

**Main Duties and Responsibilities of the Role**:

- Work and live with young people in line with their Care / Pathway Plan to ensure that the guidance and targets as laid out by Social Care are met, evidenced and completed within the relevant timescales within residential and community settings.
- Engage and motivate the young person as far as practicable, to accept and use resources effectively.
- Ensure that concerns relating to the safety and welfare of the young person are brought to the attention of the key worker, management or Senior Leadership Team immediately and escalated according to Safeguarding and Child Protection Procedures.
- Complete all Case Records, logs and reports with accurate information ensuring that information sharing and GDPR Protocols are observed at all times.
- Write reports in relation to individuals as required and engage with Multi-Agency Teams to ensure that the safety and progress of young people is supported by a 360 degree network of professionals.
- Ensure that the diverse needs of young people are met considering their ethnic heritage, religion, language, disability and that gay and lesbian young people receive a fair service based on only their needs.
- Ensure that all Health and Safety Principles are upheld to maintain the quality of the residential setting in line with the protection and safety of children, young people and vulnerable adults and abide by all SDA Care Covid 19 Regulations.
- Fully contribute and use supervision to effectively raise issues of concerns and as a tool for personnel development.
- Undertake relevant training as necessary
- To undertake any other work appropriate to the levels and general nature of the duties of the post.
- To attend any meetings in order to contribute to the effectiveness of support delivery and safeguarding.
- To undertake any training required in order enhance the personal skills and professional development, acknowledging any deficits in knowledge or practice, identifying training needs and formulating training plans with the direction of personnel.
