An exciting opportunity has arisen for an enthusiastic community practitioner to join our existing Community Learning Disability team in Northumberland. You will join a large, progressive, multi-disciplinary team at a time when our service is growing from strength to strength. The role of the community practitioner is to operate within a Positive Behavioural Support framework and conduct person centred, specialist behavioral assessments and interventions including the development of multi component Behaviour Support Plans. Close collaboration, coaching and support of carers and paid staff to increase their capacity to support others is a key focus of the role.
The post currently is based at Greenacres but involves working with clients across the county. Joint working with Community Learning Disability Nurses colleagues and other partners connected to the pathway is another key component of the role.
The Northumberland Community Learning Disability Team is a large, specialist multi-disciplinary team which provides assessment and interventions for adults with learning disabilities, their families and carers who live in Northumberland. The team is made up of a range of clinicians including learning disability nurses, clinical psychologists, physiotherapists, nurse associates and Positive Behavioural Support practitioners. We work with people who have learning disabilities and have a range of mental health, physical health and/or behaviours which challenge in addition to a learning disability. The team has 5 locality bases across the county, this post is based at Greenacres, Ashington though regular travel across the whole of the county will be required.
The role of the Community Practitioner (PBS) is to operate within a Positive Behavioural Support framework. Working with key stakeholders, you will be expected to carry out person centered behavioural assessments including, functional behavioural assessments which lead to the development of multi component Behaviour Support Plans, you will support the implementation of BSPs through coaching and modelling and evaluate the impact on the quality of life of service users. There will be opportunities to deliver teaching and training and to facilitate person centered workshops. Close collaboration and support of carers and paid staff to increase their capacity to support others is a key focus of the role.
