**Job Overview**
We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in guiding patients through the healthcare system, ensuring they receive the appropriate services and support tailored to their individual needs. This position requires strong organisational skills and the ability to work effectively in an office environment while maintaining a patient-centred approach.

**Responsibilities**
- Assist patients in navigating the healthcare system by providing information on available services and resources.
- Coordinate care plans by collaborating with healthcare providers, social workers, and community resources.
- Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with relevant regulations.
- Conduct follow-up calls to assess patient satisfaction and address any concerns or questions.
- Provide education to patients and their families regarding treatment options, available resources, and self-care strategies.
- Participate in team meetings to discuss patient progress and share insights for improving care delivery.
- Support administrative tasks within the office, including scheduling appointments and managing correspondence.

**Requirements**:

- Previous experience in a healthcare or office setting is preferred.
- Strong organisational skills with attention to detail.
- Excellent communication skills, both verbal and written, with the ability to empathise with patients.
- Ability to work independently as well as part of a team.
- A commitment to providing high-quality patient care and support.

**Job Types**: Full-time, Part-time

Pay: From £11.44 per hour

Expected hours: 18 - 30 per week
