Oxford Health and Buckinghamshire are leading the implementation of the Community Mental Health Framework to improve patients experience of care andplug the gap between Primary and Secondary Care Services. These unique roles will empower patients aged 18+ yearsto access timely and evidenced based support from Mental Health service.

The post holder is integral to accomplishing ease of access to mental health carefor Buckinghamshire’s population at primary and secondary care level in an efficient and timely manner. The successful applicant(s) will facilitate improved opportunities forjoint working with Buckinghamshire’s voluntary care sector, whilst contributing to the leadershipand development of personalised care, transformation agenda and Integrated care systems (ICS).

As part of the Single Point of Access Multi-Disciplinary Team (MDT) your role will provide mental healthcomprehensive triage, signposting and professional adviceto patients, their family and your colleagues. You will work with professionals to facilitatebrief intervention for patients, to enable recovery within the community, whilst recognising the value in your skills to ensure prompt access to secondary care specialisms as appropriate.

We are looking for autonomous, driven, and confident practitioner(s). This role provides the opportunity for continuing professional development (CPD), with scope to influence service innovation and development. This role is for practitioner(s) who pride themselves on providing ahigh-quality comprehensive triage for those identified as needing mental health support within a secondary mental health setting.
