The limb Reconstructive Fellow post is a fixed term post of either 6 or 12 months providing sub-specialist focused post-FRCS Plast (or equivalent) training in the management of Major Trauma and Limb reconstruction. The aim is to equip fellows with the skills to deliver Traumatic and lower limb reconstruction at consultant level. This post does not partake in the regular SPR on-call in Morriston, but will include on-calls for lower limb cases across both sites during working hours and there will be opportunities for the fellow to cover on-call commitments to cover staff absences/leave etc if desired. The department of plastic surgery offers a wide range of services and it may be possible to make local arrangements to gain exposure to other aspects of the service.
You will provide advice to clinicians in hospital and primary care, originating either from telephone calls, MDT discussions or ward referrals. Ability to travel between hospital/Clinic sites will be required to undertake this post. KEY RESPONSIBILITIES AND OBJECTIVES OF THE POST Detailed weekly commitments will be agreed at the beginning of the post and may be negotiated locally depending on training needs. This will include a number of Orthoplastic Trauma MDTs, outpatient clinics and theatre sessions.
Fellows will function as an integral part of the team (as dictated by skill and experience) liaising with other members of the MDT, clinicians and patients. We would expect fellows to be involved in the majority of patient management discussions throughout the patient journey, supported by senior clinicians. By the end of the post we would want fellows to have a highly developed understanding of: - MDT management of Major and Lower Limb Trauma (to include attendance in OPD of other disciplines such as orthopaedics, physiotherapy/OT and rehabilitative medicine) - MDT Orthoplastic OPD assessment of post-traumatic patients. - MDT Orthoplastic assessment/management of fracture related infection and osteomyelitis.
- Development of a management plan appropriate to the type of trauma sustained, patient function and goals. -Understanding the indications for limb salvage or amputation, with Consultant supervision. - Trauma Surgery: Patient resuscitation according to the principles of ATLS and ETC, Damage control surgery, traumatic wound debridement, advanced reconstructive techniques (inc free tissue transfer) - Primary Lower Limb Surgery: Understanding how to revascularise the traumatised limb, Ortho-plastic excision of devitalised tissue, grading of the open fractures, temporary stabilisation of bone injury, temporary management of the wound, primary amputation, advanced reconstructive techniques (including free tissue transfer); when indicated in primary surgery. - Secondary Lower Limb Surgery: Ortho-plastic wound management, definitive fixation and soft tissue coverage using advanced reconstructive techniques (including free tissue transfer), the staged management of fracture related infection and osteomyelitis after MDT discussions, revision of lower limb reconstructions.
You will actively participate in the training and supervision of junior doctors and medical students through formal and informal teaching and training opportunities. You will be expected to participate in the management, development and modernisation of the service and to support colleagues in meeting training obligations. Supporting the current orthoplastic service across the SWTN in attaining BOAST 4 compliance. Heavily involved in the MDT management of open lower limb trauma including management decisions as part of the MDT, complex surgical planning of the primary and definitive procedures, carrying out the primary and definitive surgeries with members of the orthoplastic team.
