**Job overview**:
SDEC boasts a diverse team of ACP’s who are supported by medical and nursing staff. Autonomously assessing, investigating and managing same day emergency medical patients will be your primary role; also, you will in-reach into the Emergency Department (ED) as well as supporting referrals from GP’s and NWAS. The ACP team together with their Clinical Lead and support of the Ward Manager, are committed to the ongoing success of SDEC through innovative pathway development and collaborative working.

Patient experience of SDEC is of fundamental importance to our reputation and growth; you will possess highly developed interpersonal skills and be an authentic and values based individual.

**Main duties of the job**:
Your clinical caseload will be diverse and case management should be underpinned by a dedicated approach to patient-centred care and collaborative working alongside our multi-disciplinary teams, to ensure a tailored management plan is put in place which addresses both current issues and proactively looks to prevent potential issues which may arise.

- Enhanced clinical skills-able to work autonomously
- Excellent clinical reasoning/decision making skills
- Computer literate

**Desirable criteria**:

- Adult Life support
- Experience if undertaking clinical audit

**Communication/leadership/education**:

- Evidence of highly developed communication/negotiation skills
- Promotes team working and best practices within current role
