Virtual Ward Clinician

Fixed term - 6 months

This is an exciting opportunity to work with the Sutton Virtual ward in a new role which combines clinical duties with line managing a team of Urgent Response Nurses. Working closely with the multi-disciplinary team you will assess patients in their own homes who have been discharged early from hospital or to avoid admission where possible.

You will also work within St Helier hospital when required, identifying suitable patients, building relationships and educating colleagues.

Fixed term - 6 months
- Undertake advanced holistic assessment of patients in the community or hospital environments
- Line management responsibility for a group of Band 6 nurses (sickness, performance management, training and development)
- Assisting the operational manager with rota planning, annual leave etc.
- Lead on audits and reporting as required by the operational manager
- Take an active part in MDT meetings and decision making for patients under the care of the Virtual Ward
- Work closely with the Virtual Wad Clinician's (band 8a) to ensure smooth day-to-day running of the Virtual Ward
- Take responsibility for own professional development
- Ensure a balance of clinical (to be at least 40%) and non-clinical tasks, prioritising and managing own time.
- Act as an enthusiastic and knowledgeable representative of the Virtual Ward

Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.
