Make A Difference as an Intermediate Care Support Worker (ICSW) in our Reablement Team.

We are seeking to employ Reablement Team Supervisors who enjoy working with adults aged 18+. The aim of the service is to support vulnerable adults to be as independent for as long as possible, safely in their own homes. You must be flexible and able to use initiative when putting into practice care support/ rehabilitation programmes set up alongside professional staff.

We are also about to start our exciting new collaboration with the Whiston Hospital Discharge team called Home First which we hope will speed up safe discharges from the Hospital.

Our office is Based at Nightingale House (adjacent to Whiston Hospital), our service will operate from 07.30am to 10pm, 365 days per year.

You will work 22.5 hrs on a 6 week rolling rota, including early (7.30 3.30) and late (2.00 10.00)

We will provide you with the necessary training and support to enjoy and to succeed in the role.
