Rose Ward - Assistant Practitioner -Band 4

As part of our ongoing strive for excellence we are developing a 'care team' approach around patient centred care. The Assistant Practitioner will take part in ongoing assessments, they will plan, deliver and evaluate the needs of our service users, in accordance with their Care Plans. They will take part in ongoing risk assessments, recognising and responding to any changes, ensuring that the care and safety of the service user remains the priority. The role also provides the opportunity to provide therapeutic interventions, both 1:1 and within a group setting, to further enhance the care and support offered to our service users.

You will be required to work shifts in the numbers alongside our Clinical Support Workers and Registered Nurses. You will work a combination of days and night shifts including unsociable hours i.e. some weekends and bank holidays.

Rose Ward is within Hammerton Court located on the Julian Hospital site, is a modern purpose-built unit housing three wards that provide services for men and women with dementia. The unit is configured around Beach ward, the male admission ward, Reed Ward, the female admission ward and Rose being the mixed gender longer stay, with 13 beds on each of the wards.

The Julian site hosts a total of five Norfolk later life inpatient wards which affords you the opportunity to build close networks with other colleagues, to build on their experience and knowledge of the role.

Each ward has an MDT that includes nursing team, OT, social workers, psychiatrists and psychology, physiotherapists, physical health nurses, discharge facilitators as well as pharmacists.
