Intermediate Care Advanced Assessor

Ealing Intermediate Care service has an exciting new role within the Intermediate care with a focus on the integrated discharge hub and Clayponds hospital. The successful caudate will be enthusiastic and motivated with experience of supporting enabling care, facilitating discharges and working within community services. You will have experience of working at and advanced level within rapid response or a bedded unit and must have up to date advanced assessment skills and be a active non-medical prescriber.
Within this role you will work as part of an integrated MDT within ECP and LNWHT with a focus on supporting the D2A model of care across Ealing in collaboration with NWL sector peers.
Whilst working at the Ealing hospital site you will act as trusted assessor for patients within the acute setting and completed trusted assessments for CHC settings and clerk and prescribe for patients being transferred to Clayponds Hospital.
Whilst working at Clayponds hospital you provide a senior cover function which includes; reviewing deteriorating patients and making or amending medical and care plans, coordinating and clerking admissions, coordinating and managing discharges, resolving site issues and acting a point or contact for on-call managers.
- This role will be based across Ealing integrated discharge hub and Clayponds Hospital community bed site.
- Utilise clinical knowledge and skills to perform trusted assessments for people being discharged via Discharge to Asses (D2A) pathways 1, 2&3.
- Work collaboratively with partner organisations to identify patients ready for discharge and to facilitate the discharge.
- Work within community beds setting to triage referrals and allocate admissions from across North West London (NWL)
- Provide oversight of the discharge to assess process ensuring that there are no delays to discharge and that a “home first” approach is adopted.
- To represent intermediate care on sector capacity and escalation calls.
- Work within a multi-agency environment as a advanced assessor to embed joint working and collaborative approach to D2A
- To provide operational oversight of the discharge hub and community bed setting respectively

- Integrated Discharge hub functions: - Use clinical skills to act as trusted assessor for D2A pathways 1-3
- Work with teams in respective organisations to support established discharge pathways for Ealing residents
- Act as point of contact for partner organisations
- Ensure daily reporting is completed to the agreed standard
- Support the development and embedding of SOP and BCP plan
- Represent Discharge Hub with partners in relation to operational discharge hub matters including supporting Surge and Escalation policies and procedures arising from LAEDB.
- Support and report into weekly review meetings
- Clayponds Hospital Functions: - Triage referrals using advanced assessment skills including interpreting pathology and radiology results
- Admit patients into community beds, using advanced skill such as prescribing and physical assessment
- Review and care for deteriorating patients within the community and bedded setting
- Provide operational leadership across wards and resolve issues relating to allocation, staffing and patients flow.
- Support ward and community teams functions as needed
