To work within MDT and liaise with internal and external organizations and to communicate effectively

The successful applicant will assess people referred for memory problems as well as provide interventions that support recovery, wellbeing and social inclusion. You will also act as care co-ordinator to service users referred to the multi-disciplinary team externally by GPs or by inpatient wards. The successful applicant will have good communication skills

The successful applicant will develop and negotiate care plans with service users, families/carers and other professionals, and ensure that quality and timely care is delivered at all times. You will also undertake joint work with other members of the team and external agencies providing support to the needs of service users.

The successful applicant will be able to demonstrate their clinical abilities whilst promoting high quality services for older people. In turn, we can offer regular managerial, clinical and professional support and the opportunity to continue to develop your practice through a range of learning and development opportunities
