**Who we are looking for** We are looking for a hard working and enthusiastic support worker to join our expanding team. As a support worker, you will support people to achieve their ambitions and live as independent a life as possible both at home and in the community.
Horizons' is a tenancy support service that supports young people aged 16-25. Considered as a homelessness prevention service (since 2014), it primarily supports young people to access and maintain stable accommodation, and to develop the skills and confidence for independent living. The support provided includes building a support plan and needs assessment with the young person, including their health and wellbeing, support to effectively manage their home, and to access external personal development opportunities and services in their community. The service refers or signposts young people to services providing specialist support, to overcome issues such as mental health, which may affect their ability to maintain their tenancy and to thrive.

**Role and Responsibilities**:

- Provide support that positively impacts the person we support’s health and wellbeing
- Contribute and implement support plans
- Be responsible for the delivery of all support and its administration, including personal care, tenancy support, health needs (including assistance with medication), personal development, emotional needs and inclusion.

- Qualified to the level required by the SSSC as a support worker in a housing support service and care at home service, or a willingness to work towards and to successfully complete qualification within the SSSC specified timeframe.

**Desired Skills and Attributes**:

- If you’re starting a career in social care, or are already working in social care but don’t have the qualifications just yet, we will support you to gain the appropriate qualifications needed to register without condition with the SSSC.
- We want to encourage our staff and their own personal development. Training will be provided to help you excel in your role. We offer funded SCQF levelled qualifications, such as SVQs and HNCs as part of your development plan.

**About the Service** We provide tenancy, emotional, and social support to young adults in their own homes in partnership with local authorities and housing associations. We also offer pre-tenancy support to individuals who require housing but are not quite ready to move in. We support individuals with their emotional, social and tenancy needs. Supporting them to access their local community, attend college, appointments, work, or voluntary experiences. We support a visiting support model, where we work closely with a local housing association to provide supported tenancies both furnished and unfurnished for young people who are transitioning into their first accommodation We also provide the same support through our outreach model, where individuals live in their own tenancies. Within our outreach programme, we support individuals who choose to self-manage their support needs through Self Directed Support. This role does not involve personal care.

**Additional Notes**
- Shift work, weekend work and sleepovers may be a requirement of this role. Flexible working arrangements are also available. These can be discussed at your interview.
- PVG scheme membership and SSSC registration are requirements of this post.
