An Exciting opportunity has arisen to join the passionate and friendly team on Ward 14 the Acute Stroke Unit (ASU) at Glan Clwyd Hospital. The Acute Stroke Unit is a 26 bedded ward which delivers stroke services and oversees treatment of adults of all ages. All patients are managed in the ASU during the acute phase or post thrombolysis following their stroke and remain in acute care for 48-72hours until stabilised and then commence their rehabilitation. The unit works closely with Emergency Department for rapid patient assessment. The ASU provides a complete multi-disciplinary team approach with all relevant therapies being based within the ward to allow smooth transition of care and collaborative working at its best.

A team working approach is adopted to ensure both safe practice, individualised care and a positive patient experience is created for all patients. As a HCSW you will be expected to assist registered nurses/practitioners to deliver based care to patients/clients, working in partnership with other health care professionals, the patient their family and carers.

- Ensure the principles set out in the fundamentals of care are incorporated into daily practice to ensure high quality service for patients and clients
- Assist the patients to maintain their own hygiene and personal appearance
- To perform duties within the plan of care delegated by a qualified practitioner, only for which he/she has the relevant training and assessment
- Prepare ward/clinical area for receiving patients by ensuring area is generally clean and tidy - assist in maintaining the standards in the clinical area and report any untidiness/clutter to the appropriate person.
- Participate in protected mealtime for patients by ensuring patients receive their meals/beverages in a timely and safe manner, ensuring hands are washed and the environment is conducive to mealtimes e.g. preparing bed area.
- Act as a chaperon for patients undergoing examinations or other procedures
- Maintain privacy and dignity whilst assisting in the personal hygiene and toileting of patients who are unable to care for themselves.
- Perform patient observations; temperature. pulse and respirations following a period of appropriate training and assessment
- Observe and report changes in patients' conditions to the Registered Nurse

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
