Lincolnshire Community Health Services NHS Trust provide a 24 hour home visiting service across the county of Lincolnshire.
As part of this role you will support the wider urgent care system by providing face to face assessments within the urgent treatment centres and telephone assessments via our award winning clinical assessment service.
You will be working as part of the countywide home visiting service, providing autonomous face to face care in patients homes or care homes with a focus on hospital avoidance.
You will be based at one of our visiting hubs that are located alongside the Urgent Treatment Centres where you will respond to visit requests across the county. There will be times when you will be required to work out of other bases, other than your allocated base, to ensure there is countywide cover across urgent care.
The role is a mix of solo working and working along side a Health Care Support Worker or staff members.
Bank position also considered.
Visit patients in their own homes to see and them to avoid acute hospital admission
The role requires you to be able to work flexibly, work within a team but also to be able to make individual decisions.
Shifts are 12 hours and you will receive unsocial hours payments for working nights, weekends and bank holidays. Due to the nature of the role the vast majority of shifts will be either at weekend or nights.

You will work in a variety of clinical areas. working alongside our other community services you will provide care to patients in their own homes, work alongside GP's and other colleagues in Urgent Treatment Centres, and support colleagues from the Clinical Assessment Service (CAS) who provide a hear and treat/refer telephone triage service. CAS training is available and will be offered to appointees who feel this may be of benefit to them.
