**About the role**:
Are you passionate about supporting children with disabilities and their families? Join our dedicated **Children with Disabilities Team**, a specialist social work service that provides assessment, support, and access to tailored services for children and young people with severe disabilities, as well as their parents and carers living in the London Borough of Hillingdon.

We are currently seeking a** compassionate Family Support Workers** to join our team in a full-time, permanent capacity. This is a rewarding opportunity to make a real difference in the lives of children with complex needs and their families.

**About the Role**:
As a Family Support Worker, you will play a vital role in:

- Reviewing and monitoring care packages.
- Supporting children under a Child in Need plan.
- Minimising the impact of the child's significant needs on their family’s daily life.
- Managing a caseload and maintaining regular, meaningful contact with families and professionals.
- Conducting home visits and working collaboratively with schools, health services, and other partner agencies to secure positive outcomes.

**About You**:

- Hold a qualification in Health, Child Care, Education or equivalent.
- Have experience working with children and families, ideally including children with disabilities.
- Are confident supporting children with substantial physical, sensory, or learning disabilities, severe autism, or life-limiting conditions.
- Have strong knowledge of safeguarding policies and procedures.
- Are proactive, compassionate, and solution-focused in supporting families with day-to-day challenges.
- Are committed to continuous professional development and team collaboration.
- Can build and maintain effective networks within the Council and wider partner services to enhance support delivery.
