Vaughan Thomas Ward is an 18-bed Male Psychiatric Acute inpatient ward for those who have acute or severe and enduring mental illnesses, often with co-morbidities, which require treatment which would otherwise not be able to be given in the community.

Vaughan Thomas ward aims to provide the most intensive nursing care and proactive and effective treatment to promote service users to recover and return to suitable accommodation when discharged to the community.

To actively engage service users in meaningful conversation and therapeutic activity. Specifically, gaining an understanding of the individual's interests and incorporating these into the activities that are planned.

Written documentation of your observations and interactions with service users clearly and accurately on the relevant clinical system.

To participate as a member of the team in delivering care as prescribed in the individual's care plan.

Participate in the development, monitoring and evaluation of individual care plans. This will mean contributing your opinions and actively involving yourself in conversations whereby decisions are being formulated.

Develop therapeutic relationships as part of the ongoing assessment and care delivery process, and ensure relevant information is reported to the trained nurse on shift.

Encourage and empower service users to be independent but assist and support them to carry out activities of daily living when needed.

Be aware of basic symptomatic features that require observation and monitoring, and know when to seek assistance and advice.

Escort patients with section 17 leave out of the ward. This is often on the hospital grounds but may also be outside of the grounds if the consultant permits.

Undertake and record physical baseline observation. This includes pulse rate, blood pressure, oxygen saturation, respiration rate and temperature.
