**Overview**

**Responsibilities**
- Serve as the primary point of contact for patients, addressing inquiries and providing support throughout their care process.
- Coordinate appointments, referrals, and follow-up care to ensure continuity of services.
- Maintain accurate and up-to-date patient records, ensuring compliance with confidentiality regulations.
- Collaborate with healthcare professionals to develop individualised care plans tailored to each patient's needs.
- Monitor patient progress and communicate any changes or concerns to the relevant healthcare team members.
- Educate patients and their families about available resources, treatment options, and self-care strategies.
- Assist in administrative tasks within the office to support the smooth operation of the care coordination process.

**Experience**
- Previous experience in a healthcare setting is preferred, with a strong understanding of patient care processes.
- Excellent organisational skills with the ability to manage multiple tasks efficiently.
- Strong communication skills, both verbal and written, to effectively liaise with patients and healthcare providers.
- Proficiency in using office software and electronic health record systems is advantageous.
- A compassionate approach towards patient care, demonstrating empathy and understanding in all interactions.
- Driver with own car

This role is essential for fostering a supportive environment for patients while ensuring that they receive the highest standard of care possible.

Pay: £28,000.00 per year
