Family Outreach Worker

We are currently looking for a caring and thoughtful individual to join our Swansea Intervention Team, providing support and interventions to parents with addictions. You will need to work in a non-judgmental, anti-discriminatory way at all times whilst delivering support and advice to parents/ carers and their families, who suffer with substance misuse issues and other complex needs, to improve the family's outcomes and promote recovery.
The post holder will be providing interventions for parents/carers who misuse substances and have children aged under 18 years old, and deliver advice and support under the Hidden Harm agenda. You will receive referrals from the single assessment service, carry out assessment reviews, develop and review care plans, and contribute to a holistic package of care for the whole family. The aim is to improve relationships and outcomes for the family by providing support on an outreach basis, working closely with all other organisations involved.
**To apply, you will need to posses a NVQ 3 in Health & Social Care and/or relevant professional qualification (Considerable relevant experience would be considered in lieu of formal qualifications) and hold a full valid driver's licence with access to a vehicle for work purposes.**
Post Outline:
Undertake substance misuse assessment reviews, conduct risk assessments, and develop and review care plans to meet identified needs of parents/carers and their children.
Identify appropriate service to meet identified needs.
Work collaboratively with the parent/carer to reduce alcohol and drug related harm and refer for substance misuse treatment as appropriate.
Engage with parents’/carers’ children who are substance users/misusers to minimise the harm they are causing themselves, their families and the wider community.
Develop close and collaborative relationships with a wide range of service providers, both in statutory and voluntary sectors to ensure the holistic care of the family.
Promote health awareness to parents/carers and their children.
Undertake appropriate administrative tasks.
Maintain timely and accurate client records utilising the WCCIS, case management system.
Provide advice, support and training to colleagues in partnership agencies as necessary.
Person specification:
NVQ 3 in Health & Social Care and/or relevant professional qualification. Considerable relevant experience would be considered in lieu of formal Qualifications.
Experience of working with families
Experience of delivering services to substance misusers
Experiencing of working with the harm reduction model
Experience of liaising with specialist and non-specialist agencies
