**Job summary**:
We are looking for experienced staff from a health and leisure background to support our new programmes of care. If you feel passionate about helping individuals regain their confidence following a change in their abilities then we would love to hear from you. You will be supported by experienced hospital and community therapists and will follow guidance and or complete assessments of individuals in their own or health care setting to maximise their return of function.

Full training and induction will be provided, you will receive regular 121s, be part of a new peer support programme and be changing the way we deliver NHS services.

We deliver support across the new forest and have several opportunities. If you are interested please make contact.

**Main duties, tasks & skills required**:
Implement and design rehabilitation programmes for individual patients and groups as advocated by Physiotherapists, including home exercise programmes

Assist and conduct exercise/rehabilitation classes both onsite and off both independently and with assistance of Physiotherapists and Physiotherapy Assistant

Assist patients to complete exercise or rehabilitation programmes with physical assistance and or guidance as required. Such as practising standing from sitting or regaining confidence in walking.

To advice reablement carers of delivery of exercise programmes and appropriate progressions following reassessment.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

**Qualifications**:
**Essential**:
Full & Valid Driving Licence

Level 2 Gym instructor

Level 3 or 4 Personal Training Diploma or equivalent

First Aid Qualification

Member of the Register of Exercise Professionals

Appropriate Liability Insurance

Or

Otego qualification

Equipment ordering and assessment training.

Proven delivery of community balance and falls prevention classes.

**Desirable**:
Background in anatomy, biology, exercise physiology and sports biomechanics.

Broad range of other qualifications (Exercise To Music, Circuit Training, Resistance equipment and free weights, Obesity and Diabetes Management)

Foundation degree in Health and social care

**Experience**:
**Essential**:
Proven teaching experience.

Evidence of group and individual exercise instruction.

Experienced in designing individual and group exercise programmes.

At least one year experience of fitness or sports coaching. Or x4 years experience in delivery and providing rehabilitation support within a community or hospital setting.

**Desirable**:
Experience working with complex patient in a community or home setting.(essential of not from a Personal trainer background).

Keen interest in physiotherapy/occupational therapy rehabilitation or health care provision.
