Purpose of the Role:
The Care Assessor is responsible for carrying out comprehensive assessments of individuals requiring domiciliary care services. This includes developing personalised, outcome-focused care plans that promote independence, dignity, and quality of life while ensuring compliance with regulatory and organisational standards.

**Responsibilities**

Conduct thorough assessments of new and existing clients, identifying their care and support needs in line with CQC requirements and organisational policies.

Develop person-centred care plans that detail how identified needs will be met, incorporating client preferences, goals, and risks.

Review and update care plans regularly or when there are significant changes in a client’s condition or circumstances.

Liaise effectively with clients, their families, advocates, healthcare professionals, and other stakeholders to ensure a holistic approach to care planning.

Complete risk assessments (e.g., moving and handling, medication, environmental) and ensure appropriate mitigation measures are in place.

Support the care team in understanding and implementing care plans to deliver safe, effective, and compassionate care.

Maintain accurate and timely records in line with data protection and confidentiality requirements.
- Participate in quality assurance activities, audits, and service reviews to ensure continuous improvement.
- Keep up to date with legislation, best practice guidance, and developments in adult social care.

**Qualifications**
- NVQ/QCF Level 3 (or above) in Health and Social Care (or equivalent).
- Experience in care coordination or supervisory roles.
- Knowledge of CQC Key Questions and Quality Statements.

Skills and Experience:
**Essential**:

- Experience in domiciliary or social care settings.
- Strong understanding of person-centred care and the Care Act 2014.
- Excellent written and verbal communication skills.
- Ability to conduct detailed assessments and produce clear, accurate care plans.
- Good IT skills, including experience with electronic care management systems.
- Ability to work independently and manage workload effectively.

**Job Type**: Part-time

Pay: £27,000.00-£29,000.00 per year

Expected hours: 20 per week
